To prepare for your first visit at Northeast Georgia ENT, please complete the patient forms below and bring them to your appointment. Please download the patient intake form, then print it out to complete it and bring it to your appointment. You’re also welcome to complete this form in-office by simply arriving 15 minutes before your scheduled appointment time. You can also download and review the HIPAA form, but you will sign this form in our office.
Patient Portal
Our secure Patient Portal, ModMed (Modernizing Medicine), gives established patients convenient access to their health information and a secure way to communicate with our office. Once you've had your first appointment and become an established patient, you can request a portal invitation by calling our office at (770) 536-4352 or by submitting the Contact Us form below.
For your privacy and security, protected health information (PHI) is only shared through our secure patient portal. Please do not send personal health information through the Contact Us form or regular email.
